Respite
Respite-Individual (-10hrs) Respite-Day (+10hrs)
Respite services are provided on a short-term basis, because of the absence or need for relief to those persons who normally provide care for the participant. Respite may be provided in the participant’s home/private place of residence or the private residence of a respite care provider.
Supported Community Connections (SCC)
Supported Community Connections (SCC) will be provided by Peak Supportive Services in a variety of settings in which the Member interacts with persons without disabilities, in addition to those individuals who are providing services to the member. SCC services include:
- Socialization opportunities to facilitate and build relationships and natural supports in the community.
- Assistance with learning adaptive skills
- Personnel to accompany and support the individual in community settings.
- Resources necessary for participation in activities and supplies related to skill acquisition, retention, or improvement.
- Structured physical activities chosen around health and physical wellness, encouraging participation, and promoting healthy habits of exercise and creativity.
- All settings will offer opportunities that address the member’s interests in learning, health, goals, social needs, and skill development.
